Travel Clinic

Ask us online through our Digital Front Door

You can now request Travel vaccinations via our digital front door, if you are registered at our surgery, by clicking on the logo below.

You will be asked for your NHS number and date of birth for data protection reasons and we will send you a form to fill in.

If you are not registered at Ashfields you can still request travel vaccinations by visiting the practice and asking for a hard copy of the form.

This form has several questions to fill out that our nurse team will process before contacting you on the next steps.

Please make sure all travel forms are submitted to the surgery at least 8 weeks before your travel date, as our nurses need enough time to access your request and administer your vaccination course.

If you have less than 8 weeks left before you travel you may have to go to a private travel clinic that specialises in short course immunisation.
