There is a dietetic out-patient clinic at Ashfields Primary Care Centre which runs on a weekly basis.

What is a Dietitian?

Dietitians are highly specialised practitioners who translate scientific information about food and nutrition into practical dietary advice to promote health and treat disease.  They are registered with the Health & Care Professions Council (HCPC) and are qualified to treat a range of medical conditions with diet therapy, developed to meet individual needs.  They provide patient specific advice which is practical, safe and based on current scientific evidence.

How can I be referred to a Dietitian?

Patients can access the service via their GP, practice nurse or hospital consultant. The clinician will refer you to the service providing the dietitian with medical information.

What to expect at your first appointment

The initial appointment will last approximately 30 minutes. The dietitian will take a thorough diet history and ask health-related questions, including your current medication. If the dietitian thinks that you would benefit from a follow-up appointment, you will be provided with a date prior to leaving the clinic. A subsequent appointment will last approximately 15 minutes.

What if I am unable to attend my appointment?

If you are not able to attend your appointment, please contact the reception desk as soon as possible so they can reschedule.

Neurology

Consultant Neurologists from Stoke Hospital run a weekly out-patient clinic at Ashfields seeing both new and followup patients. The clinic is open to all not just Ashfield patients and it is run as part of the normal out patient service with a GP referral needed.

Midwives

Midwives from Leighton run ante-natal clinics from Ashfields rooms. Book with the midwifery service direct.

Audiology Clinic

Ashfields is host to a hearing clinic. The service is provided by Industrial Diagnostics.

Patients from the age of 55 years and older can be referred to the service by their GP, if they fit the referral criteria.  Patients will be seen at Ashfields Primary Care Centre, however the company also provides hearing assessments at home for our housebound patients.

AAA Screening

Abdominal aortic aneurysm (AAA) screening is provided at Ashfields Primary Care Centre. Patients who have been diagnosed with an AAA, via an ultrasound scan, will be referred for screening.

Who Provides the Service?

NHS vasectomy clinics are held at Ashfields Primary Care Centre on Tuesday mornings. The service is headed two of our doctors, Dr Rosson and Dr Gowda, who provide this service.

Who can Refer to the Service?

Clinic referrals are accepted from all practices within the South Cheshire CCG and Vale Royal CCG areas, therefore you must be registered with one of these surgeries prior to your GP’s referral.

How can I Arrange a Referral?

The consultation with your doctor at your practice is the counselling process. Your doctor will discuss the vasectomy procedure and answer any questions you have. If you are happy to continue with your request for a vasectomy, your referral will be send to Ashfields.

If you are a patient at Sandbach GPs and you don’t have any questions that you would like to discuss with your doctor, you won’t need to book a routine appointment to see your GP first. You can read our Vasectomy Patient Booklet and call our direct line below to book your Vasectomy.

During your consultation your doctor will provide you with written information. This booklet provides more detail about the procedure. This includes post-vasectomy care and how to provide a sample. How do I Arrange the Appointment?

Patients are responsible for arranging their own vasectomy appointment. 

Once you have read the above booklet you can call Ashfields Vasectomy Line on:

01270 446568 between the hours of 10:00-12:00 and 14:00-16:00 to book your appointment.

Please note: if you are employed in a heavily manual capacity (especially lifting) we recommend refraining from returning to work for the rest of the week. If you work in an office or have light duties you should be able to return to work the following day.

What Should I do on the Day?

Your checklist:

  • Shave your scrotum (this reduces the risk of post-operative infection)
  • Make sure you have suitable painkillers available at home
  • Make a note of any questions you may still have
  • Have breakfast (it helps to avoid feeling faint during/immediately after the procedure
  • Wear supportive underwear

What Happens Afterwards?

Your checklist:

  • Take the specimen pot, form and instructions home
  • Submit you specimen sample at 12 weeks (please refer to the instructions)
  • Use alternative contraception until you receive a letter informing you that the procedure was successful.

Ultrasound Clinic

Details

Ashfields hosts a community non-obstetric ultrasound service (NOUS). This service is provided by an experienced team of sonographers from Diagnostic Health Care.

Patients are referred to the service via the e-Referral System, which allows them to select an appointment of their own choosing.

Other benefits of the community-based clinic include:

  • Much shorter wait times
  • Faster reporting of the results
  • Reduced pressure on hospital clinics
  • No parking fees

There are conditions where patients will still be referred to the hospital for an ultrasound scan; these include:

  • Obstetric dating scans
  • Scans for soft tissue problems (breast, ganglions, thyroid, etc)
  • If a patient is under 18 years of age

Diabetes Clinic

Appointments are available from Monday to Friday with a Diabetic Nurse.

All patients with diabetes will be offered an annual review appointment. Patients are required to see a Health Care Assistant (HCA) prior to their annual review with a Diabetic Nurse. The HCA will ask you a number of health-related questions, take a blood test, measure your height and weight, check your blood pressure and will ask for a urine sample (bottles are available to collect from the reception desk). You will also be asked to see the Diabetic Nurse in two weeks’ time following the date of your assessment.

During your appointment with the Diabetic Nurse, you will have the opportunity to talk about any problems you may have and to discuss your individualised treatment plan. The Diabetic Nurse will perform a foot risk assessment and agree a plan of care with you, including subsequent appointments.

Patients are able to contact the Surgery if they have any diabetic related queries in between planned appointments; the Diabetic Nursing Team endeavor to answer queries with either a face-to-face review or via a telephone consultation. GP appointments are also available, if required.
